Looked After Children Health Advice

Description

Provision of health assessments to Looked After Children and aged between 6-18.
Health promotion and education for all looked after children and young people.
Working with designated doctor and Clinical Governance department of PCT to ensure best practice.
Provision of advice and support to residential social workers, carers and fellow professionals.
Advice and support to children and young people looked after by other authorities and their carers.

Eligibility

 

Children and Young People Looked After.
Social Workers, residential Social Workers and other allied professionals.
Foster carers, parents and family members.

 

Procedure

 

Direct contact (self referral) to the Life Chances Team.

Referral from Social Worker, Carers, Health or other professional.

Appointment for health assessments, home visits sent to children and young people.

Drop in sessions for residential units

Referral on to other services eg. Hearing screening, Family Planning, Speech and Language if necessary.
