Older people and people with a disability who have difficulties with daily living tasks can receive advice, simple equipment or more complex adaptations to the home if required. The Equipment and Adaptations Service can visit you at home to discuss any problems you may be having. The first step to accessing the service is to complete the on-line referral form or telephone the Service and request an assessment. You will be asked about yourself and the difficulties you are having. If you prefer, you can ask a friend or relative to contact us on your behalf. Every request is considered individually and people in unavoidable high risk situations are seen as soon as possible.
Service users who are interested in undertaking their own self assessment and purchasing their own equipment from an independent supplier are recommended to vist the self assessment website.
The Equipment and Adaptations Service cannot provide equipment based on a SARA assessment. This is because an assessment by a member of staff from the Equipment and Adaptations Team is required to access equipment from this service.
