Care Programme Approach

A framework for ensuring that people experiencing mental ill health get the best possible help towards recovery.

Assessment of health and social needs of people accepted into specialist mental health services.
The formation of a care plan developed from the needs identified in the assessment.
The appointment of a key worker (called Care Co-ordinator) to keep close touch with service user and to monitor and co-ordinate care.
Regular review and where necessary agreed changes to the care plan.

The service is available for people in receipt of Specialist Mental Health Services.

Procedure

Referral to the Community Mental health Team in the same locality of the GP to the individual.
