Leaving Hospital

Support available on discharge from Hospital setting.

The Nurse looking after you will refer you to the hospital Social Work Team for an Assessment or if you already are in receipt of services ,a request or a reinstatement of service will be made -- to ensure a safe discharge home.

The Government has asked all Social Services departments to review criteria for assessments to make sure that all service users have equal access to Social Care ServicesTherefore the Fair Access Care Services criteria will determine what level of support you require on discharge from hospital

In line with Fair Access to Care Services guidance Social Services will assess plan and provide care for adults who have needs that fall within the agreed criteria the eligibility criteria agreed in Barnsley consists of two bands. Since April 2003 your needs will be met if you are in either the critical or substantial band following the community care Assessment.

There maybe a charge for services dependant on your income and the hours of support you will receive.
Normally would purchase HOMECARE STAMPS from the POST OFFICE.

Getting help from Social Services

Response Times / Next Steps

The Social Work Team will respond to the request for Assessment within 48 hours once the discharge date from Hospital has been agreed at the Multidisciplinary Team meeting this involves both Health & Social Care staff.

Charges/Fees

You will be charged for social care support provided by Social Services in your own home. The charge will be determined by the level of support you receive and your income. Your Social Worker/Care manager will advise you further during the assessment .

Method of Payment

Payment can be via purchase of homecare stamps from post office or cheque.
