Leaving Hospital

 Support available on discharge from Hospital setting.

The nurse looking after you will refer you to the hospital Social Work Team for an Assessment or if you already are in receipt of services, a request or a reinstatement of service will be made to ensure a safe discharge home.

The Government has asked all Adult Social Services departments to review criteria for assessments to make sure that all service users have equal access to Adult Social Care Services.  Therefore, the Fair Access Care Services criteria will determine what level of support you require on discharge from hospital

In line with Fair Access to Care Services guidance Adult Social Services will assess your plan and provide care for adults who have needs that fall within the eligibility criteria agreed in Barnsley this consists of two bands.  Since April 2003 your needs will be met if you are in either the critical or substantial band following the community care Assessment.

Response Times / Next Steps

The Social Work Team will respond to the request for Assessment within 24 hours once the discharge date from Hospital has been agreed at the Multidisciplinary Team meeting this involves both Health & Social Care staff.

Charges/Fees

You will be charged for social care support provided by Adult Social Services in your own home. The charge will be determined by the level of support you receive and your income. Your Social Worker/Care manager will advise you further during the assessment .

Method of Payment

Payment can be via purchase of homecare stamps from post office or cheque.
