The Physical and Sensory Disability Teams provide assessment, care management and social work support for physically disabled people and a specialist service for people with sensory impairments. Social Services is working with partners in the Primary Care Trust and other agencies, referring on where necessary, for example, to physiotherapy or occupational therapy. Services that may be provided after an assessment include day services or outreach; enabling support at home including direct payments; short-term breaks.
Physically Disabled people aged 18-64 who have a substantial and permanent disability.
People with sensory impairments of all ages.
Eligibility for services is determined by Fair Access to Care Service through council policy.
Access to services is through a community care assessment. Each person is assessed against the current eligibility criteria.
Assessment's are generally carried out in a person's home and will involve the individual, those who care from them and relevant professionals. The result is an agreed plan identifying the individuals and carer's needs. This is called 'The Care Plan' and it is reviewed on a regular basis.
Unless the situation is an emergency, services will not normally be provided before an assessment has been carried out. If you are already receiving services and your needs have changed, you should ask for a review or reassessment to be carried out.
Anyone can request an assessment for himself or herself or someone they know who is having difficulties. Simply contact your local Social Services office, the Disability Team, or someone from the Health Service such as you G.P, a District Nurse or Health Visitor who you may already see.
