If you suffer from a long term condition, you'll have your own way of coping with it, but taking care of your overall health and wellbeing can put you more in control of your condition so that your quality of life is improved.
Barnsley Telehealthcare and Care Navigation Service helps people with long term conditions to manage and monitor their health.
Working with South West Yorkshire Partnership NHS Foundation Trust, they offer:
- a free small electronic device at home that can help you and health professionals to monitor your condition
- personalised information and advice
- support to help you improve your wellbeing
- help to get in touch with local services
The service is free and you can access it in person or over the phone, providing you meet certain criteria.

Contact South West Yorkshire Partnership NHS Foundation Trust on freephone 0800 612 1976. They'll take your details and ask a nurse to call you when it's convenient.
You can also talk to your GP or specialist nurse about this service and ask them to refer you.
