Getting your independence back

If you've been in hospital or had a period of illness and you need some help to live at home, you may qualify for support from our reablement service.

What the service provides

The service provides short-term support, for up to six weeks, to help you to regain living skills and independence.

As part of your support we'll give you advice and guidance on the best way to manage your day-to-day activities safely. We'll also provide you with small items of equipment, or advise you about suitable technologies such as pendant alarms, that would make life easier for you.

Our reablement service is registered with the Care Quality Commission which means that we have quality inspections. You can read our latest inspection report on their website.

How to access the service

The service is free and you can be referred by any agency that's already supporting you, such as the hospital or social work team.

Following acceptance of your referral, we'll assess you to make sure that any support we provide is designed around your needs.

What happens when the support ends

When your period of support ends we'll talk to you about any ongoing care and support needs you may still have and refer you to the appropriate services.
